Thank you for contacting the Family Fertility Center. I am pleased you chose to be a part of our Gestational Surrogate Mother Program. Your interest in helping an infertile couple achieve their dream of having a child is extremely important and you are to be congratulated.

It is important to evaluate the appropriateness of your participation in the Surrogate Mother Program before you make a commitment. Our web site contains many articles, and medical information that will help answer many of the questions you may have regarding becoming a gestational carrier. If you feel this is something for you, please call us here at Family Fertility Center on our toll free number at 800-939-6886 or locally at 925-977-4850.

After a short phone interview we will give you access to the on line gestational surrogate application; this is an extensive questionnaire for you to complete. The questionnaire, together with head and shoulders color photos of you and your children, will form your application. Please fill it out carefully and truthfully. Prospective Parents will make their choices based on the information they see in this application. The people looking at these applications will receive no personal information about you other than your first name. Please complete the enclosed authorization for release of information and return it with your application.

There is also a form for input from your partner (if appropriate), which should be returned with the application. We also require that a copy of your driver's license and birth certificate be submitted along with your application.

We recommend that you make a copy of your application before mailing. Occasionally an application is not received or is lost in error. I look forward to hearing from you.
