Dear Intended Parent(s),

I am very pleased to have this opportunity to introduce you to our Surrogate Mother Program. I have included our Mission Statement/Resume, Fee Schedule, and Flow of Events-Surrogacy. I have included my resume because you are hiring me to help you build your family. I consider your decision to work with me on one of the most important projects of your life as an honor. I regard the trust you place in me as a wonderful treasure and promise to honor it with complete sensitivity and professionalism.

Please read the “Flow of Events-Surrogacy” form found at our web site. Please schedule a telephone appointment with me after you’ve read the “packet” information so that we can review this material and I can answer your questions.

I will work closely with either a physician of your choice or one of our referral physicians who specialize in these types of procedures. Family Fertility Center will schedule treatments, arrange for testing, disburse fees, and act as liaisons on an “as needed” basis. We have a strong case management program so that we can serve you until your child is born.

Please call our center for a list IVF physicians and attorneys that we work very closely with. They can help you review your Service Agreement with FFC and your Surrogate Agreement between you and the surrogate couple with whom you are matched.

Please contact me for either a telephone consultation or a two-hour office visit to discuss how Surrogacy and FFC can help you build your family. This intake appointment includes a consultation with me, and the option of receiving up to six copies of Surrogate profiles. A fee of $200.00 will be collected before the meeting starts.

If you live out of the area and/or cannot come into the office we can do a telephone conference consultation after we receive your $200.00 fee. We’ll review the six profiles that you have chosen and the ones I’ll select. Please have a list of questions available for our conference call. It will be your responsibility to initiate the telephone call.
