Flow of Events--Surrogacy

Intended Parent(s) will meet with Director of FFC to discuss process and explore working together in an initial consultation. There is a $200 fee for this intake appointment which will be collected (in cash or by check) at the beginning of this appointment. (Please note that FFC will not accept credit card payment for any fees.) At this time, Intended Parents may review example surrogate applications. If referral for any medical program is needed, it can also be provided at this time. An MMPI psychological test is administered to both Intended Parents. Test is scored by Dr. Becking who will separately bill Intended Parent(s) for examination.

After Intended Parent(s) MMPI test is evaluated by Dr. Becking, Intended Parent(s) will send signed "Service Agreement Gestational Surrogacy" contract to FFC and pay 1/2 the current center fee. Also, at this time, FFC will request the sum of $5,000.00 to open a Surrogate Expense Account. When FFC provides an ovum donor at the same time, then current FFC "Service Agreement Ovum Donation" contract is also signed by Intended Parent(s) and FFC Director. Fees for Ovum Donation Center Fee and Donor Expense Fund are collected at the same time. Please refer to Donor Flow of Events for other fees and procedures.

FFC will assist Intended Parent(s) to prepare videotape. This videotape will act as an initial introduction between the Intended Parent(s) and a potential Surrogate.

The Matching process with prospective Surrogates begins once Dr. Becking has evaluated the MMPI test for Intended Parent(s) and "Service Agreement Gestational Surrogacy" is in place and respective fees are collected.

• Intended Parent(s) select Surrogate.
• The Surrogate is contacted by FFC who will describe the Intended Parent(s) to her and the Surrogate will decide if she would like to help the Intended Parent(s).
• If the Surrogate agrees to help Intended Parent(s), her and her partner's MMPI tests are scored by Dr. Becking.
• After Surrogate and her partner pass the MMPI test, videotapes are exchanged by mail, after verbal authorization for release from all Intended Parents and Surrogate and partner. Consent forms authorizing release of videotapes are mailed with videotapes and must be signed and returned to FFC.
